Imaging records are now available through Clearpath! Instead of picking up a CD, you can access your records from the comfort of your home.

How Clearpath benefits you

• You can instantly share these records with any doctor or family member.

How to get Clearpath
If you have been recently seen at Visalia Imaging, you will receive an email when your records are ready, including your Clearpath login information.

If you need prior records, please follow the steps below.
1. Simply Create an Account at patient.myclearpath.com/login
2. You will be asked to enter your First and Last Name, Date of Birth, E-mail, and Time zone.
3. Once you create an account you will get a temporary password sent to your e-mail
4. Use the temporary password and your email address to log in
5. Request your records

How to Request Records
1. Once logged into Clearpath, select the Request Record Tab
2. To request a record, select the Request Record button on the bottom right corner
3. Select Record Custodian (Visalia Imaging)
a. If you do not see the facility’s name listed, please reach out to help@myclearpath.com
4. Select records to request, Body Part, and date Range
5. Agree to the terms and conditions and submit
* Prior to requesting your records, please ensure a copy of your Photo ID is attached to your profile under the My Identity Tab

**Once submitted, the facility will review and process. This may take up to 48 hours. Once request is processed, if approved you will see your images under the My Records Tab**
